This is an INTERNAL FORM for FIABA organizing staff only to input people into our communications system. This form is only for people who should receive communications in English.

"Geographic team" refers to the best match for the team that is organizing that person. For example, if someone lives in Daly City but got connected through a church in SF based on the work that the SF team is doing, put Daly City under "City" but San Francisco under geographic team that the person is organizing with.

"SMC Network" is for the people who live in SMC and want to get communications but they don't live in a city with a directly impacted organizing team and they aren't in touch with/in relationship with any of the directly impacted organizing teams.
