As the country tries to put COVID in the rearview mirror, nursing home workers are seeing things get worse, not better, for both them and their residents.

Residents are not getting the quality, dignified care they deserve. Workers are burning out and leaving the profession, making the staffing crisis even worse.

We need national staffing standards for all nursing homes so workers can go home at night knowing our residents got the care they deserve.

I give my consent to Service Employees International Union (“SEIU”), Change to Win, and their successors, assigns, Locals, and Affiliates (collectively, the “Union”) to use and publish the information I have provided. I give the Union permission to use and publish my name, voice, photograph, video image, and/or any statements I have made. The Union has my permission to publish this material in print, on the Internet, other TV and radio, or through any other media worldwide in perpetuity. I waive any right that I may have to inspect or approve any finished products incorporating this material. I agree to release the Union from all liability and any claims that I may have arising out of the use, publication, or disclosure of this material. To the extent any of these materials are copyrightable, I further grant permission for the Union to copyright this material, and I release the Union from any liability arising from any blurring, distortion, alteration, optical illusion, or composition of my photograph or video image. I acknowledge and agree that the Union shall have sole ownership rights to any images and video of me and comments made by me which are contained in any printed material, videos, digital and online media, advertisements, and any other material, and hereby relinquish and waive any ownership interest which I have in such material, to the extent that any such interest exists. I further acknowledge that the Union may freely assign any and all of their rights described in this Release to other entities allied with the Union in the campaign to raise wages and improve working conditions for health care workers.

I am at least 18 years of age and have every right to grant these permissions and releases in my own name. I have read and reviewed this Release and Verification. I confirm that all information in the material that I have or will provide to the Union is true and accurate, to the best of my knowledge.
