Breaking Barriers Keystone Awards: Nominations Form
Thank you for nominating someone for the Breaking Barriers Keystone Awards! We’re thrilled to celebrate the incredible change-makers in our community. These awards will be presented at the 2025 Breaking Barriers Conference. The award categories are as follows:
- Youth Leadership
- Health Wellness Trailblazer
- Educational Leadership
Before you proceed with your nomination, please review the guidelines:
1. The application portal closes at midnight on December 31st.
2. Incomplete applications will not be considered by the committee.
3. You may submit only one nomination per individual, but that individual can receive nominations from others.
4. Nominations must come from a career professional in or relative to the nominating field. School/University Staff and local officials may also offer nominations.
