Co-Thrive (formerly the Collaborative Community Response Initiative - CCRI) started as a coalition of primarily healthcare providers and public health professionals exhausted by the status quo, dreaming about how we could deliver health more collaboratively. Over five years of dreaming together, bringing partners into the fold, and partnering with committees exploring solutions to the various aspects of the systemic and community-level issues folks were facing, we formalized an organization and started applying for and receiving funding to try out our collaborative prototypes across the state.

We developed this playbook to help communities build a formal infrastructure for collaboration where learning and iteration can begin with the goal of creating the conditions, at all levels, that make it possible for every Coloradan to achieve whole health and thriving.
