This Reflection & Feedback form serves 2 main purposes for us:

1. We use the demographic information for writing grants and securing funding
2. We use the reflections & feedback to be accountable to our community, and help us create the most accessible, fulfilling, safe and healing content possible

None of the demographic questions are required and are completely up to you if you'd like to answer. If you are able and willing to fill it out, we deeply appreciate you sharing it with us. We understand and honor your boundaries.

We also ask for testimony or affirmation about the event. This is also used for grants, sponsors, and our website to show the impact of the work we do, and to secure funding to continue that work. Testimony is also completely optional, and we're so grateful for whatever you're able to share with us.
