This form is to be completed by parents of student participating in activities of Schools’ Climate Action Network (SCAN).

The purpose of this form is to ascertain your views regarding the creation of, and use of photos/images which include your child, which might be used internally by SCAN; online on our website/social media or in the print media.

As you are aware, there are potential dangers associated with the posting of personally identifiable information on a web site as it is not possible to control who may access such information. As an organisation we seek to find a balance between these dangers and the wishes of students to publish their campaigning and learning activities undertaken as part of SCAN on the internet.

It is the policy of SCAN not release any personally identifiable information without consent from the student in question as well as from you as a parent or guardian. Personally identifiable information includes student names, photo or image, residential addresses, e-mail addresses and phone numbers. It is our policy to keep contact information (residential addresses, e-mail addresses and phone numbers) as well as information about where your child will be ( such as their plans to attend an event or meeting) strictly confidential.

We may, however, want to include photos, videos or names on our website and publications as well as in newspaper articles, and thus seek your permission to do so.

In this form you will be asked to respond to three questions:

I/we GRANT permission for a photo/image of my child and their name to be published on the Schools’ Climate Action Network website or other media

I/we GRANT permission for a photo/image of my child without their name to be published on the Schools’ Climate Action Network website or other media.

I/we GRANT permission for photos of my child to be used internally in the SCAN community such on in internal online communication platforms and presentations that will not be published on the SCAN website.
