This pay rise won't be won by just a few people getting involved and active. It is going to take all of us if you want that 15%. This form is for people who want to take it to the next level and say how they plan to do it.

To win we need you to:

• Put yourself forward for handing out #NHSPay15 badges and commit to speaking to your colleagues in your workplace
• Get involved with organising a demonstration in your area on the 12th September
• Steward your local demonstration
• Recruit more activists at your next event

We'll support you with training to get this done, you just need to decide if you've had enough and are ready to do something about it!
